All calls will appear as a fully native option. According to a 9to5Google report, the feature has been tested to work successfully for the Samsung Galaxy Watch 5 and the Google Pixel 6A.

For the voice calling feature to work on the Samsung Galaxy Watch 4, users will need to update WhatsApp to the beta version 2.22.19.11. Once updated, the calls will appear with a WhatsApp logo under the contact information, which was missing in the case of the Galaxy Watch 5. Meanwhile, WhatsApp voice calls appeared as a fully native option with no distinguishing logo when tested for the Pixel 6A.
The WhatsApp icon seen on the Galaxy Watch 4 follows the One UI theming, which means that this may only appear when paired with a Samsung Galaxy device. While a previous WearOS update would only allow you an option to “Answer call on phone”, the latest one will let you take calls directly from your wrist as long as you are paired within range. The update is currently being rolled to beta testers and is available by default.
